The patient with metastatic cancer with no obvious primary site has always been a special and difficult problem for the practising physician. We report a case here who presented as a metastatic adenocarcinoma to the thyroid and the cervical lymph nodes. The extensive diagnostic studies, which were not conclusive at first, disclosed the fact that the primary site was the ovary during the long course of the disease. The patient is still alive at the 14th month since the diagnosis and according to our literature review this case is the only reported metastatic adenocancer to the thyroid and the upper cervical chain with unknown primary in which occult primary malignancy was ultimately detected at the ovaries.
